What this policy covers
This policy covers fulcrumboulder.com and the three forms on it: the appointment request on Book, the question form on Contact, and the newsletter sign-up in the footer. Your medical records are covered separately by our Notice of Privacy Practices, which we give you at your first visit.

What we collect
What you type into a form: your name, email, phone number, the area of pain you choose, the times that suit you, how you plan to pay, and any note you add. We also count visits to each page and the site that sent you, without your name.

How we use it
To answer your question, schedule your evaluation, check your insurance before the visit, and send the monthly article if you asked for it. We do not use form details for advertising.

How long we keep it
Requests that do not become a visit are deleted after 12 months. Once you become a patient, your details move into your medical record and follow the record keeping rules for health care providers in Colorado.

Cookies and analytics
The site uses privacy-friendly analytics that set no advertising cookies. You can block cookies in your browser and every page still works.

Your choices
Ask us to show, correct, or delete what you sent, and unsubscribe from the newsletter from any email we send. Write to hello@fulcrumboulder.com and we reply within 30 days.
